The Aim is to assess of the 1 year results of differentiated treatment of patients with neuroischemic form of diabetic foot syndrome (DFS). Materials and methods. Clinical register of 285 patients with neuroischemic form of DFS (2015–2017) was analyzed. Division of patients on degrees according Texas university classification was carried out. All patients received the differentiated outpatient treatment and spesialized surgical treatment (including endovascular procedures — 52). Results. Result is unknown for 29 patients (10,2 %) in the group. Among 256 others: epitheliazation — 121 (47,3 %), preservation of DU — 53 (20,7 %), small lowerextremity amputations — 47 (18,4 %), high lowerextremity amputations — 35 (13,7 %). Conclusion. The regular observation of patients in DFOP and differentiated treatment at the stage of specialized surgical care allows keep the supporting limb for patients with neuropathic foot in 99 %, with neuroischemic foot — in 86,3 % cases within 1 year of follow up.
CEAP clinical classes C4–C6 of chronic vein diseases (CVD) are characterized by dermatological manifestations: hemosiderosis, atrophie blanche, varicose eczema, lipodermatosclerosis, healed and active venous ulcer. All these changes are the result of veinspecific inflammation and seem to be complications of venous hypertension. We recommend using the term “complicated forms of CVD”. The diagnostic of these conditions is not easy. There are more than 50 diseases which can lead to leg ulceration and about 20 diseases resembling varicose eczema. Algorithms for diagnosis and treatment of complicated forms of chronic venous diseases are proposed.
In this paper the new author's formation conception of schizoid structure of psyche, new clinical scales of schizoidia and regression, types of neuropsychic defect, which determines schizoid register of disorders, were described. Author suggests various energy regimes and levels of function of neuropsyche. The new concept of schizotypic disorder was proposed in this article.
Establishing the specific type and cause of stroke requires the use of imaging techniques and traditional clinical examinations. Cases of ischemic stroke, not detected using neuroimaging techniques, are more often recorded among women, localized in the trunk, not associated with the presence of diabetes and high scores on the NIHSS scale. Possible overdiagnosis of stroke in the vertebralbasilar basin in the presence of only vestibuloataxic syndrome at admission to the hospital. Stenosing and occlusive atherosclerosis of the BCA and a history of atrial fibrillation did not affect the detectability of ischemic stroke.
The purpose of the study is a comparative analysis of the structure of typical complications and fouryear survival of two types of radialcephalic arteriovenous fistula (AVF) in the “anatomical snuffbox” and the Brescia-Cimino type. Materials and methods. 323 patients with endstage renal disease were analyzed, with 358 radialcephalic AVFs were made. Group I — 53 patients with a radialcephalic AVF in an “anatomical snuffbox” (AVFAS) (n = 53). Group II — 270 patients with a radialcephalic AVF of the Brescia-Cimino type (n = 305). The initial cumulative patency of the AVF in both groups was evaluated at 12 and 48 months Kaplan-Meier. Results. In group I, 8 thrombotic complications were revealed. Produced 4 effective reconstructive operations. In group II, 136 complications were identified. Thrombosis — 123 cases. Cumulative patency after 12 and 48 months in group I was 90.33 % and 52.12 %, in group II 76.95 % and 46.69 % (p < 0.05). Conclusion. The radialcephalic AVF formed in the “anatomical snuffbox” has fewer complications (p < 0.05). The primary cumulative patency of these AVFs within 48 months is higher than in the AVF group of the Brescia-Cimino type (p < 0.05).
The article deals with the problem of computer games in adolescence as a form of addictive behavior with the subsequent formation of dependence. The stages, causes and risk factors of gaming addiction, as well as the psychological classification of computer games with the description of features of occurrence of addictive disorders taking into account specifics of interactive technologies.
The analysis of anesthetic management, intraoperative infusion and the postoperative period is presented on the example of 105 patients operated on for abdominal aortic aneurysm in the period from 2013 to 2018. It is concluded that the isolated regional anesthesia prevails in the studied group of patients. This is due to the predominance of the “closed” method among the conducted operations — endovascular aneurysm repair (EVAR). Most of the “open” surgical interventions were performed under combined anesthesia. During 5 years of analysis, the volume of intraoperative infusion therapy decreased. In 2013–2015, hemo and plasmatransfusion was performed in 60–100 % of cases, whereas in 2016–2018 — only in 20 %. The same applies to colloidal solutions. This is associated with reduced duration of surgery and a decrease in the volume of intraoperative blood loss. Patients whose regional component was present in the anesthetic management, on average, spent less time in reanimation. Due to prolonged epidural analgesia, the restoration of all body functions and activation occurred earlier than with analgesia narcotic analgesics. The time of hospitalization for patients who underwent EVAR are two times less than the time of hospitalization of patients who underwent “open” surgery. The highest percentage of complications is in the group of patients who have undergone “open” surgical interventions.
Objective: To evaluate the immediate results of emergency operations for dissection or rupture of the aneurysm of the infrarenal aorta, depending on the tactics chosen. For dissection and rupture of the infrarenal aorta, surgery in a specialized unit is preferable. The decision on transportation is made by the vascular surgeon after examining the patient. When detecting acute ischemia of the limb, performing crossbypass surgery avoids burdening the patient's condition, preventing loss of the limb.
