Abstract

Unfortunately, surgical activity is inevitably associated with the development of a certain number of postoperative complications. Endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (EST) are minimally invasive but technically demanding procedures with a relatively high complication rate and multiple risks. In recent years, public attention to the results of medical activities and the number of lawsuits filed against doctors has been steadily growing in all countries. In such conditions, the development of a complication after ERCP/EST, which has all the external signs of iatrogenic, may serve as a basis for complaints for medical negligence. Although administrative and legal claims to charge doctors with malpractice are usually based on poor treatment outcomes, the decision of the patient or relatives to file them is often dictated by factors other than medical ones. The paper analyzes the main ethical and medico-legal consequences of complications, as well as the proper actions of a surgeon or interventional endoscopist after their appearance.

This will ensure the formation of general professional competencies and an increase in the quality of training of future doctors who are able to carry out their activities in the context of health care reform. Aims: to study the effectiveness of the use of cases in practical classes for students in the study of the discipline “Pathophysiology”. A comparative analysis of progress according to the intermediate control of knowledge in the exam and in solving cases in practical lessons by students of the SUSMU specialties General Medicine, Pediatrics, Dentistry in 2018/2019 and 2019/2020 academic years was carried out. An average increase in the qualitative performance of students in the specialties of General Medicine, Pediatrics, and Dentistry when applied in practical lessons cases was established.

Abstract

In Russia annually about 450 thousand cases of acute cerebrovascular attack which in 35–50% come to an end with a lethal outcome are registered. The main cause of acute cerebrovascular attack is atherosclerosis of the brachiocephalic arteries. The paper highlights the history of the formation of reconstructive interventions in the carotid basin — from the study of atherosclerosis of the main arteries of the brain to carotid endarterectomy and stenting. Over the past 50 years, brachiocephalic artery surgery has made a significant leap in development. The article compares the results of different methods of carotid artery reconstruction: classical and eversion carotid endarterectomy. The advantages and disadvantages of each of the methods of reconstruction are described, indications and contraindications are presented. Brachiocephalic artery surgery is the field of interest of doctors of many specialties: vascular surgeons, neurosurgeons, X-ray endovascular surgeons, cardiac surgeons and radiologists. Foreign studies have not revealed statistically significant differences in the results of surgical treatment of these patients. The main factor influencing the outcome of the operation and the frequency of complications is the experience of the surgeon (10 operations per year). The prevalence of atherosclerotic lesions of the carotid arteries forces neurosurgeons to participate more actively in brain revascularization.
