Use mesh repair in surgery strangulated abdominal wall hernias
Abstract
The urgency. Treatment of the restrained hernias of a forward belly wall remains a serious problem modern emergency to surgery that is caused high mortalities — in Russia, agrees to data of different authors, it makes from 2 to 9.5%, and the number of relapses after plastic of hernial gate local fabrics reaches level of 33%. The question by definition of indications to use of mesh materials and their kinds for liquidation of hernial defect is not solved up to the end, especially at a resection of a loop of intestines in urgent situations.
Goal: to improve the treatment results of patients with anterior abdominal wall hernias have been infringed.
Material and methods. The analysis of direct results of treatment of 431 patients with the restrained hernias of a forward belly wall at which is presented used technology with application using a Prolene mesh.
Results. Patients with restrained groin hernias was 146 (32.3%), with umbilical — 143 (31.5%), postoperative ventral hernias — 85 (18.9%), a white line of a stomach — 34 (7.5%), femoral — 30 (6.8%) and other localisations 13 sick (3%). After 24 hours from the disease beginning 108 patients (24%) have addressed. Necrosis guts at its infringement in the hernial collars, demanded its resection were at 44 patients (9.6%). Postoperative complications are noted at 72 patients (15.8%). Arose seroms in a zone of implantation of a grid — at 56 patients (12.4%) More often, wound suppurations are noted at 12 (2.7%), the postoperative pneumonia was at 4 sick (0.7%). Has died after operation of 9 persons (1.9%).
The conclusion. The discussed technique of treatment is enough safe, economic technology and conducts to improvement of direct results of treatment.
