Abstract

The aim o of the study is to assess the trends and forecast of the epidemiological process of HIV infection among women in the Chelyabinsk region. Materials and methods. Data from the reporting forms of the Chelyabinsk Regional Center for the Prevention and Control of AIDS and Infectious Diseases, data from the ASODOS electronic database software, data from epidemiological investigations. Results and conclusion. The data obtained demonstrate an increase in the impact of HIV infection on the mortality rates of the working-age population, especially in the context of a declining population in the region. At the present stage, a decrease in the incidence of HIV infection in a highly affected region has been established. This fact is largely associated with the widespread use of antiretroviral therapy. However, mortality rates tend to increase, due to the insufficient effectiveness of measures aimed at attracting this group of patients to medical observation and treatment.

Abstract

Relevance: The basis of antibiotic resistance control is qualitative monitoring. Modern online resources solve the problems of information capacity. Such a resource is the online platform AMRcloud. Purpose: to create a local system for monitoring antibiotic resistance using the AMRcloud online platform in the Clinic of the Federal State Educational Institution of the Southern State Medical University. Materials: data from the report of the microbiological laboratory of the Clinic on the number of microbiological studies conducted, isolated isolates, etiological structure for the period 2021- 2022. Results of implementation: Since August 2022, the AMRcloud online platform has been introduced into the work of the Clinic of the Federal State Educational Institution of Higher Education in YUGMU. Conclusion: Implementation of the AMRcloud online platform into multidisciplinary hospital operations enables the creation of a local evidence base on antimicrobial drags activity and antimicrobial resistetans epidemiology.

Abstract

The aim of the study is to research the frequency and clinical manifestations of cryoglobulinemia in a population of patients with chronic hepatitis C at different stages of fibrosis. Materials and methods. The source population is 23492 patients with HCV who are registered at the dispensary according to the Register of the Chelyabinsk region. The studied population was randomly formed – 100 patients with verified HCV. As part of the ongoing single-stage clinical study, standard general clinical and biochemical studies (bilirubin, ALT, AST, GGT), HCV RNA in blood with subsequent genotyping of the virus, qualitative and quantitative analysis for serum cryoglobulins (cryocrite level), instrumental methods of research (ultrasound, transient liver elastometry with determination of the degree of fibrosis on the METAVIR scale) were performed for each patient. Results and conclusion. Cryoglobulins were found in the blood of 63 patients (63%). At the same time, with stage 4 fibrosis in 14 (22%), with stage 3 in 8 (13%), with stage 2 in 1 (1.5%), with grade 1 – 17 (27%), with stage 0 – 23 patients (36.5%). Of the total number of patients, only 22 (35%) had complaints and clinical manifestations of cryoglobulinemia. In the vast majority of cases, hemorrhagic purpura was recorded – 18 (28.5%), volatile arthralgia - 4 (6.5%). Clinical manifestations of mixed cryoglobulinemia were observed in 35% of patients with HCV (hemorrhagic purpura, arthralgia). A reliable correlation was obtained between the level of cryocrit and the female sex with a pair correlation coefficient of 0.237 (p<0.05). The data obtained indicate the need for constant monitoring of the frequency of mixed cryoglobulinemia in patients with HCV, early etiotropic therapy with direct antiviral drugs.

Abstract

The aim: The article reports about problems of iron deficiency correction in chronic heart failure (CHF). Material and methods: It were described the forms iron deficiency that exist independently or in combination. The indeterminacy of Russian clinical recommendations in patients with anemia and reduced ejection fraction is discussed. Results and conclusions: The characteristic of CHF patients with anemia was given on basis of our own research – absence of reticulocytosis, erythropoietin (EPO) levels heterogeneity, possibility of excessively high gormone levels and decreased levels of progepsidin in blood with iron deficiency in the body. It was confirmed the necessity of using EPO and hepcidin with ferritin and transferrin saturation to choose therapeutic tactics. Four clinical solutions variants are suggested. It was paid attention to development of general longterm program of therapeutic and preventive measures that are aimed at metabolic resources replenishment, increasing iron absorption from the gastrointestinal tract.

Abstract

The arm of the work: to analyze the pharmaceutical market of proton pump inhibitors in the Chelyabinsk region. Material and methods. An analysis of sales in the context of income in the Russian Federation and in the Chelyabinsk region for the period from 2018 to 2020 was carried out. Results. An increase in the incidence of acid-dependent diseases and an increase in sales of proton pump inhibitors have been established. In the pharmaceutical market of the Chelyabinsk region, the representation of proton pump inhibitors is sufficient, among them the most demanded molecule is the molecule of omeprazole.

Abstract

Comorbid pathology aggravates the underlying disease, leads to a change in the clinical picture and becomes a complication leading to death. To assess the prediction of the quality of life and the risk of an adverse outcome in patients with comorbidities, various scales and indices for measuring comorbidity were considered. А comparative analysis of methods for measuring comorbidity frequently encountered in Russian studies is presented, presented by a modern scientific work describing methods for studying comparable diseases. The Charlson index is the most common and convenient method for predicting cases of comorbid pathology.

Abstract

The aim of the study was to evaluate the current range of para-implant complications after metal osteosynthesis and joint prosthetics and to study the features of the course of the wound process with various methods of their treatment. Material and methods. The materials of a comparative analysis of 64 clinical cases of paraprosthetic and paraimplant complications are presented, in the treatment of which various modern approaches were used to influence the inflammation focus. The results of cytomorphological studies revealed two types of inflammatory reaction of paraimplant tissues, one of which is dominated by the processes of purulent inflammation, and in the other there are manifestations of the granulomatous nature of the tissue response. Analysis of local immune reactions revealed a direct correlation between the level of phagocytosis activity and the proportion of wound macrophages, lymphocytes, or polynuclear cells, depending on the local management of the wound process. A higher activity of cellular immunity during treatment under vacuum therapy determined a 1.4-2.9-fold reduction in the elimination of pathogens and the transition of the wound process to the stage of regeneration, and was also accompanied by a decrease in the proportion of multinucleated cells and lymphocytes in the granulomatous type of inflammatory reaction. Conclusions. The revealed effects of locally controlled negative pressure exposure open up possibilities for cytoregulation and control of reactions of purulent and granulomatous inflammation and improvement of the results of treatment of paraimplant complications in purulent surgery and traumatology.

Abstract

The aim of the study is to teach the characteristics of the level of melatonin in the different nature of the pain syndrome and the chronobiological type of women with ovarian endometriosis. Material and methods. A prospective cohort study of 62 women with ovarian endometriosis was performed. The level of 6-SOMT (ELISA), the severity of pain (VAS), the qualitative characteristics of pain (the McGill questionnaire), the chronobiological type of women (the Horn-Ostberg test modified by Prof. S.I. Stepanova) were determined. Statistical processing - IBMSPSSStatistics 19. Results. The level of melatonin in women with pain syndrome and ovarian endometriosis depends on the chronobiological type. the lowest value of melatonin in ovarian endometriosis and the presence of pain in women of moderate morning and moderate evening chronotypes.

Abstract

The aim of the study is suggest a system of teaching the Russian language for foreign students of the specialty 31.05.01 "General Medicine", which would allow students to master the Russian language in sufficient volume necessary for studying medical disciplines. Materials and methods. A combination of interdisciplinary and contextual approaches. Description of the experience of teaching Russian as a foreign language, implemented on the basis of the Department of Foreign Languages with the Latin language course of the South Ural State Medical University. Results and conclusion. The system of teaching the medical Russian language to foreign students developed by us will ensure the comprehensive development of professionally significant skills and abilities in all types of foreign language speech activity. It will give them the opportunity to continue their medical education, lay the foundation for further self-improvement of the level of Russian language proficiency.

Abstract

The aim of the work is to give a medical and social characteristic of patients treated for chronic venous diseases (CVD) of the lower extremities in an outpatient phlebological center.
Materials and methods. A selective retrospective analysis of medical records for 2015, 2018 and 2021 was carried out in 303 randomly selected patients. For satisfaction assessments additionally analyzed 509 questionnaires filled out by patients of the center.
Results. Among patients with CVD, 75% were women (mean age = 50.2 years). 82% were persons of working age, among them patients with higher education made up 48%. At the initial phlebologist's appointment and after ultrasound, objective signs of CVD were not detected in an average of a fifth of patients. The most frequent classes in the registration of HSV were C2 and C3 (2015 – 77.8%; 2018 – 74.4%; 2021 – 67.6%). The share of running classes (C4 – C6) did not exceed 10%. The proportion of those treated with telangioectases and reticular veins (C1) is about 20%.Among the appointments after the examination of specialists, the most frequent was the recommendation to perform EVLC of subcutaneous veins: 2015 - 50.6%; 2018 – 36.7%; 2021 – 52.1%.The proportion of patients satisfied with the results of treatment in different positions was 87- 100%.
Conclusion. The data obtained will be used to improve the organization of receptions of phlebologists and the work of medical staff.
