Современные концепции профилактики и лечения кровотечений из варикозно расширенных вен пищевода и желудка у больных циррозом печени (по материалам консенсусной конференции BAVENO VII, октябрь 2021 г.)

Цель обзора. Описать современные концепции профилактики и лечения кровотечений из варикозно расширенных вен пищевода и желудка (ВРВПиЖ) у больных циррозом печени (ЦП) в соответствии с рекомендациями консенсусной конференции Baveno VII. Основные положения. При отсутствии клинически значимой портальной гипертензии для первичной профилактики кровотечений из ВРВПиЖ целесообразно этиологическое и неэтиологическое лечение ЦП, тогда как её наличие служит показанием к назначению неселективных β-адреноблокаторов (НСББ), среди которых препаратом выбора является карведилол. НСББ, а также эндоскопическое лигирование (ЭЛ) и трансъюгулярное внутрипеченочное портосистемное шунтирование (ТИПС) могут быть применены с целью профилактики рецидивов кровотечений из ВРВПиЖ. Для лечения острых кровотечений из ВРВПиЖ рекомендованы фармакотерапия вазоактивными препаратами (терлипрессин, соматостатин, октреотид), ЭЛ, эндоваскулярные методики и ТИПС. Заключение. Объективная и точная стратификация риска кровотечений из ВРВПиЖ позволит разработать индивидуальные стратегии их профилактики и лечения, что улучшит прогноз и выживаемость больных ЦП.
