Abstract

Increasing the availability of medical care through the introduction of new medical organizational technologies and improving the system of providing medical care to the rural population are considered priority areas for the development of the industry.

The purpose of the work is to analyze and summarize the available data on organizational technologies developed and implemented in practical healthcare for providing medical care to the rural population.

Results and conclusions. The development, selection and implementation at the regional level of organizational technologies for providing medical care to the rural population requires a differentiated approach and should depend on the dynamics of medical and demographic indicators, geographical, economic features of the region’s development and strategic directions for reforming the industry.

Abstract

The purpose of the study: to review the literature on the topic of the spread of HIV infection published in open sources.

Materials and methods. Articles by various authors published in the public domain.

Results and conclusion. The article presents materials on the chronology of the development of ideas about HIV infection. Information on the spread of HIV infection in Russia is presented. The issues of the economic burden of HIV infection are touched upon.

Abstract

The article deals with the epidemiology of pancreatic pseudocysts, the principles of diagnosis and treatment. Some classifications of pseudocysts are presented, on which tactical decisions regarding surgical treatment are based. Based on the literature data and on our own experience, the advantages and disadvantages of various interventions for this pathology were evaluated.

Abstract

The aim of the study is to identify and systematize the coping strategy of patients diagnosed with colon cancer.

Materials and methods. 47 people aged 29 to 79 years (mean age 61 years) took part in the detection, the incidence of malignant neoplasms emanating from the intestinal epithelium (colon cancer), of various stages with a pronounced differentiated tumor. All respondents found at least 3 months from the oncologist from the moment of diagnosis, which was observed in the territory of the Chelyabinsk and the Chelyabinsk region, they are on inpatient treatment at the Chelyabinsk Regional Cancer Center of Oncological and Nuclear Medicine. To collect empirical material, we used the psychodiagnostic survey method “Methods of Coping Behavior” by R. Lazarus (adapted by T. L. Kryukova).

The results of the study of coping behavior in patients with colon cancer, that the predominant coping strategy is “self-control”, and fever is pronounced “acceptance”. The coping strategies “escape-avoidance”, “problem solving planning” and “positive reassessment” are at a fairly high level of expression, while the level of expression of the coping strategies “confrontational coping”, “distancing” and “search for social support” is below average.

Conclusions. The analyzed coping strategies make it possible to predict the patient's behavior during treatment. Data on this basis have been obtained for the development of a program for the development of coping behavior in patients with a type of oncology.

Abstract

The article is devoted to the analysis of ultrasound assessment of the depth of endometrial tumor invasion into the myometrium and the identification of factors for underestimating or overestimating the depth of invasion using ultrasound in the Chelyabinsk Regional Clinical Center of Oncology and Nuclear Medicine.

Objective: to determine the capabilities of transvaginal ultrasound in assessing myometrial invasion in endometrial cancer and to determine the main factors for underestimating or overestimating the depth of endometrial cancer invasion into the myometrium at the preoperative stage using ultrasound.

Materials and methods. 130 cases with endometrial cancer were analyzed. The results of transvaginal ultrasound examination were compared with intraoperative macroscopic examination and histological findings.

Results and conclusions. The accuracy of ultrasound assessment of myometrial invasion was significantly influenced by: tumor size; the presence of myomatous nodes or adenomyosis nodes that deform the uterine cavity and their combination; minimal invasion of 1–2 mm, which is not detected by ultrasound scanning; uterine position and image quality; thickness of the mid-uterine structures; volume of the uterine body; performing an ultrasound immediately after invasive intrauterine procedures; examination during menstruation and/or uterine bleeding; unsatisfactory visualization of the internal genital organs with severe obesity and cicatricial adhesions in the pelvis.

Abstract

Parasitic myoma is a pathology considered to be iatrogenic, most often appears after laparoscopic surgery with morcellation. There are articles in foreign literature with recommendations on the use of a protective container during laparoscopic operations. But in the current domestic surgical and gynecological clinical guidelines, this is not necessary. Without it, it is possible to separate very small fragments of the neoplasm while maintaining the ability to function at the implantation sites due to vascular invasion with the omentum, walls or pelvic organs. The article provides its own description of observations on the diagnosis of parasitic myoma after myomectomy.

Abstract

We studied the dynamics of relative skin vascular density after laser infrared exposure (970 nm), continuous wave, 1, 3 and 5 W on the anterior abdominal wall of Wistar rats. 1 hour after laser irradiation, the relative density of abdominal skin vessels increased in all experimental groups, while a significant decrease was observed in the back skin. At the observation period of 1 and 5 days, the relative abdominal skin vascular density in the 3 W and 5 W groups remained above the control level. The maximum values of the relative vascular density were observed in 5 W group at all periods. Thus, the power of 5 W is preferred for further experiments to study the effect of high-intensity laser therapy on the state of microcirculation, since it causes the most pronounced long-term vascular reaction.

Abstract

The aim of the study: to identify the features of clinical manifestations of the combination of varicose veins of the lower extremities and osteoarthritis of the knee joints.

Materials and methods. The study involved 125 patients suffering from varicose veins in combination with osteoarthritis of the knee joints over the age of 40 years. The average duration varicosity was (12.7 ± 6.8) years, and for osteoarthritis of the knee joints (7.9 ± 5.4) years. The severity of pain syndrome was assessed using a visual analogue scale, the Lequesne index, and the presence of manifestations of synovitis according to ultrasound data.

Results of the study. Pain syndrome in patients with phlebogonarthrosis is characterized by increased pain in the knee joint in the evening hours, both at rest and with movement. In addition, ultrasound signs of reactive synovitis of the knee joint are significantly more common in these patients.

Conclusions. The presence of concomitant pathology aggravates the clinical picture, leads to increased pain in the evening and at night, and increases the incidence of synovitis of the knee joint.
